  • FinalForms for the 2024-2025 school year is now available to be completed online.
  • IHSAA Physicals must be completed on or after April 1, 2024, to be eligible for the 2024-2025 school year.
  • If you completed FinalForms for the 2023-2024 school year, simply update your/your child's information and upload the new physical signed by the doctor.
  • If you do not have access to a computer or wifi, please contact the TJMS office at (219) 531-3140.

IHSAA Physicals must be completed ON or AFTER April 1, 2024

for the 2024-2025 school year. 

You may access the physical the following ways:

  • https://valparaiso-in.finalforms.com/
  • Visit the TJMS office, door #1 at TJMS, and pick up a copy of the IHSAA physical form that must be filled out by your doctor or you may print it after completing the Final Form process.
  • IHSAA Physical form for the doctor.

Students-athletes may not practice, train, or compete until the IHSAA Physical Evaluation Form and the TJMS Physical Packet are completed on the FinalForms website and approved by the TJMS office.
