Travel Release Form

It is the strict policy at TJMS that if a student does not intend to ride the bus from a competition/meet/game, a travel release form is filled out. Each form must bear the signatures of the following people:

All forms must be signed and turned in to the coach at least 24 hours before the date of the competition. Any athletes who do not submit the proper forms must ride the bus to and from the event(s). A form must be filled out for each event!
