Athletic Physical Packet

 

Students wishing to participate in a sport at TJMS must complete the physical form below. Physicals must be renewed every year. Physicals may be used for multiple sports. You must complete the entire packet and return it to the main office before the beginning of your season. Incomplete physicals will not be accepted.
